Description

Data entered into different parts of the system (ie General Information and PhilHealth? Information) were not cross-checked. Two different birthdays were inputted in each and the system did not detect the mismatch.

no patient birthdate entry has been required in the PhilHealth? forms. The birthdate entry will be the actual philHealth member where the patient act as dependent.
